Roof makes sense, perhaps underneath the lining, so invisible till the odd loose one works it's way out over a few weeks.
JZA80 Supras have these exact same (type of) nuts holding the plastic covers under car that cover the fuel lines.
Looks like the nuts used to hold the rear door handle on a Discovery. Maybe they used the same for the Defender?

Hmmm... If I am remembering correctly (it's highly possible that I'm not), these look like the nuts that hold the spare wheel carrier on the rear door.
Mine was coming loose on the 90 once, so I removed the trim to tighten it up. I was surprised to see how small the nuts were compared to the weight it carries and was even more surprised as to how little torque it took to strip one of them. As a result, it's not longer got a spare wheel carrier on the back. Well, a roof rack carriers 2 spares better than one on the back anyway.
Maybe try removing the rear door trim to see if anything is missing from your spare carrier?
These are a common industrial fastener mostly used in volume assembly operations. The permanently attached washer saves one movement by the assembler, and also prevents memory lapses where the assembler does not fit a washer. Found in many locations on most mass produced motor vehicles and on white goods and all sorts of machinery.

Should we start a slush fund and order one of every nut in a defender and see what it is?
to date i have found 1/2 doz nuts -bolt same or similar to yours ,puma 110 -2010,,,none showing any witness marks of ever being tourqued up ......so.....i suspect someones sense of humour...allthough it does not appeal to me ,,when in the boondocks
