if you do some searching on the forum you should find the drawings for a defender dual spare carrier. You could take that to your local engineering fabricator and they should be able tp knock one up for you.
Does anyone know of who manfacturers the Dual Spare Wheel Carrier for the Defender 110? I've seen one around Fremantle and seems like a good way to carrier that 6th wheel (if required)...

Unfortunately, they only make a single wheel carrier for the 110 / Defender, see at the bottom of this page:

All the swing away parts can be bought, the Defender has the huge advantage of having the rear crossmember RIGHT THERE in your face
Easy to make your own, and it'll be WAYYYY cheaper.

I see cewilson found the drawings I was refering to earlier.
rijidij has also put up some pics of a project he put together for a single wheel carrier. He found a good source for the bearing/hub to get the pivot. With a bit of imagination and a tape measure I am sure that you could come up with a dual wheel carrier.
But as others have said, Why? For the times you need a sixth wheel you could find a storgae location for the trip. I used roof bars and put the spare up there.
I can't quite remember who to credit for this (needless to say it wasn't me), but here is a CAD drawing of a dual spare wheel carrier that I found on the forum some time ago.
If you do decide to use it, I'd check all the mesurements just to be sure it fits.
