This defines the true meaning of a cluster ****.
We examined this as a safety scenario and the string of causality when I was in the RAAF.
The US navy did however learn some bitter lessons from this, in regards to fire fighting and crew training, ship design, munitions handling and safety procedures.
Pity it ALWAYS costs lives to achieve improvements.
damn lucky they didnt sink the ship.
