Microsoft's ICE : Microsoft Research Image Composite Editor (ICE) (http://research.microsoft.com/en-us/um/redmond/groups/ivm/ice/)
I was introduced to this via Gary Luhm's site:

For some time I've been using other software such as Hugin and Canon's Photostitch to put together photos but this one is extraordinary.
It works with the usual panning shots but also with tilt shift lens movements and I am hard pressed to find the seams.
Apple users will be disappointed.
