my D4 is going into MLR on Thursday for its 40,000k services and likely end of warranty service.
Is there anything I should be looking our for or asking for ?
Also, thoughts on an extended warranty ? I like driving it, wouldnt ming keeping it for another 2 years.

I had my last service before warranty ended last month and when I went to pick it up they stung me for $1800! The blood pressure was still high, miso when I enquired about extended warranty I let go a bit of an expletive when I was told $3800 for two years, so I walked out the door without getting it.
But extended warranty is like insurance. If you have it and something happens, it was the best thing you ever did. But if your car has no problems, then you've done all that money.
It's all about risk and what you can afford.

I would make sue that any current or prior Technical Service Bulletins relating to known issues or upgrades are addressed - I went for a similar end of warranty service at a main dealer and they did not mention anything about the 1st gen air compressor and the upgrade kit. Finding this out whilst in a remote area outside of the most competent and less expensive ( you'd think Sydney dealers fleece you ) has been very inconvenient. Even to have had this highlighted would have been $400-600 well spent.
