My D4 has 19" wheels with 265 tyres.
The spare is 18" with a higher profile tyre meaning the overall diameter is about an inch larger than the wheels/tyres on the car.
Is this difference an issue if I need to use the spare?

That much bigger yes.
Either purchase a 18 with a similar ( within  a couple of mm) or get a 19" rim and tyre. 
What pattern 19" rim do you have?
I've got one for sale in the markets section and another pattern one is there also.

Thanks mate,
I am currently running Pirelli Scorpion Verde which are strictly road oriented and need replacing.
Considering the Pirelli Scorpion ATR's.
The D4 is my wife's daily driver and doesn't go off road at the moment but the ATR's will at least allow us to drive on gravel roads etc without the fear of damaging a low profile tyre at $700 a pop or even worse, a rim.
My car is a Troopy with mud tyres for the serious stuff.
I will see if I can keep the current spare wheel and fit a tyre to match the overall diameter.

My spare is the same colour.
If I can match the overall diameters I'll just keep the 18" rim as a spare.
Just in case that doesn't work out, how much are you selling the wheel for?

According to my maths I shouldn't have an issue.
I'll have to drop the spare, put it next to the others and have another look.
255/60 R18 overall diameter should be 763.2mm (spare).
255/55 R19 overall diameter should be 763.1mm.

Yes that is fine, a 285/60 R18 would be a different story.

And another is it a 2.7 or 3l?
If it is a 3l,and an LR 18',it may not fit.
If it is a 2.7,all good:)
Had another look at pic of wheel and brakes,looks like a 2.7?

Now I'm curious, what is the difference between 2.7 and 3.0 regarding wheels?

3.0 has bigger brakes.... LR 18" rims wont fit.

3.0L has larger brakes and 18" LR rims won't fit.
 
If I was you, i'd be getting another 19" rim or going to 18" rims for better on/offroad tyres, you can now get mud tyres in 18" now AND 18" tyres are cheaper than 19".

2.7's can go down to 17".... whereas 3.0's can only go down to 18".. (special rims from Green Oval)...

Thanks all.
I'm not going to spend a chunk of money to change to 18" rims. 
This car doesn't go seriously off road so doesn't need mud tyres, I have a fully set up Troopy for that.
All it needs is ATR's for the occasional gravel journey.
The use of the car may change in the future and if so I'll readdress the wheels then.
I doubt the cost of changing rims would be offset by the small difference in tyre prices over how ever long I have the car.
I will consider another 19" rim as a spare if I find one cheap and the postage isn't a killer, but realistically with the overall diameter very close to the same will it be an issue?

Or if things change,i would go to 17's,and keep the 19's for the road,or flog them off.
