Could it be as simple as that.

Hunt begins for the cause of the deadly Colombian plane crash. (http://thenewdaily.com.au/sport/football/2016/11/30/colombian-plane-crash-cause/?utm_source=Responsys&utm_medium=email&utm_campaign=20161201_TND)

Something to do with the flight being at the extreme edge of its range, and having to fly around bad weather.

Unbelievably, on the news tonight, just before crashing, the pilot radioed they had run out of fuel. Hard to believe.

they didn't have the required fuel reserves.

Hope endures, even as Chapecoense coffins flown home (http://thenewdaily.com.au/news/world/2016/12/03/chapecoense-survivors/)

This is a scandal of the first order. A real can of worms.

Seems that she worked out 'what' was going to happen...and 'who' was going to be blamed. . .

Whatever, I thought the Captain/Pilot was the final responsible person regarding loading, FUEL, and navigating to a distant destination.

Company Policy & Expectations,,, would also exert a certain influence.
