OMG! How on earth does that happen?
Quite easily... Several ways..
First way. Head gasket fails between oil gallery and water jacket.. When cold an engines oil pressure is 40+ psi dependant on engine, and coolant will be under no pressure whilst the system is cold.. Oil pressure forces oil into water system.
Second way.. Radiator has inbuilt oil cooler, and similar scenario when cold, oil pressure is higher than coolant pressure. Again, oil finds its way into cooling system.
Then, when the coolant is checked first thing in the morning or when its under slight pressure, the first thing that comes out of the cap is oil, as it floats on water... :)

on some cars the radiator cap looks a lot like the 710 cap.
