D4 3.0L Rear Brake Pad Question - is this normal or a dodgy pad?

Hi All,

I had my rear pads and discs replaced about 3months (nearly 10,000km) ago. Some minor squealing on initial touch of the brakes since then but nothing out of the ordinary. For context I consider myself moderate on brake application and the last sets have both lasted about 45k - 50k km each.

About 2 weeks ago the squealing got harsher and louder and then a week ago I noticed a scoring in the disc about 1cm from the hub per below - it's deep enough to be visible and catch a finger nail:

After pulling the outer pad off I saw what looks like a piece of wire / metal that seems to have been embedded in the pad when manufactured - wondering if anyone has come across this before? The pads are Ferodos.

I will reach out to the workshop that did the brakes for me so they can investigate themselves but given the time of year and concerns there may have been something stuck in there I wanted to have a look now.

I had something similar years ago with pads on a Cortina. Shocking noise not long after I'd changed them and one had a large piece of metal like substance embedded in it. Retailer virtually told me it was hard luck, he wouldn't do that today as I'm much more likely to take action (legal) if things don't come up to scratch.
Seems to have been a problem during manufacture of the pads.

Looks like a faulty pad to me, get your money back or have the workshop replace f.o.c

Pads fine it's had somthing stuck in there to groove the disc and the pad material has then worn to the disc.

Funny I have one pad with that grainy appearance on the D2. (LH front). They are TRW Lucas pads.

At odd occasions I get a noise just as I am pulling up for the last say 6 feet. It only happens when the car is hot after driving for say 20Km at least and is not consistent. It does it only sometimes.

I took the pads off, inspected them, and realised that I had put them in wrong sided as the chamfer was on the front not the back. Ground a chamfer on the back( rather than swap them around)no change
I thought that it must be the hub so replaced it. No change.
Changed the front driveshaft ,no change.
Pressure bled the brakes. No change.

The car pulls up excellently with no pulling side to side, and I believe that mine is one of the better D2s as I have a high firm pedal. I exercise the HDC every month or so. My TC works excellently.

I was watching a video of some D2s on the OTT and they all groaned and vibed when going slowly down drop ins.

This led me to think that somehow some pads are not maybe cured properly and are prone to this grainy appearance as usually my experience with pads is that they have a shiny appearance once worn in.
This seems to have few negative effects other than a noise.
I haven't changed them as they are hardly worn and seem to work well.
