It happens more often than people realise.
They take off the safety chain before the boat is in the water and only rely on the winch strap or cable when reversing or forgetting to attach it on retrieval.

I saw it happen once at a boat ramp at the river above Bateman's bay where the pub is.
Happened right next to me. scared the **** out of me..

I suspect it is people not wanting to get their feet wet [emoji23]. You get your feet wet removing the safety when the boat is immersed sufficiently to float off or drive off...just the way it is. There are boat latch devices to alleviate this, but I would still be driving with the chain on to the point where I could stand just above the water line and then remove the safety and allow the boat to rest on the latch for the last few metres (but not on a very steep ramp).
