Everything they could do wrong, they did. Complacency, incompetence, lack of training, came together to lose a warship in refit. Thankfully there was no loss of life. I feel I can comment on this as I was the RAN shipboard fire fighting instructor[ for a year] , and also served on two destroyers and a patrol boat , in refit. This is difficult reading, with young sailors lives put at risk by a failure of command.
