    How did it actually happen?

Did it just go under load, or were wheels off the ground and then it went when they grabbed again?
Not really sure, but I think that it was when there was a transfer of power to the back due to lack of drive from the front

So, the Disco has been sitting in the naughty corner long enough. Going to go and pull the diff out now. Will post up some pics of the damage when I get the bugger of a thing out.

So, diff is out. Not looking to good here.... The center has exploded and it appears that there is also some damage to the crown wheel as well. Which is not good news.

The housing has really taken a beating

Looks like a fairly typical failure of the cross shaft. The cross shaft on later models (Series 3 - on) has a circlip either end. This allows the shaft to rotate, which wears both the shaft hole in the diff and the circlips, which eventually fall off, allowing the cross shaft to go on holiday.

Look on the bright side, now you have an excuse to put in a heavy duty C&P, possibly an ARB locker as well. Hopefully a bit of panel beating and welding is all that is required to fix the housing.
