I used to work for a company who manufactured those larger types of breakers, air circuit breakers and moulded case circuit breakers. Anything from 50 amps up to thousands of amps.

We were given product training, and were shown a number of videos like this, enclosed breakers going bang in a big way. Serious stuff.

Got shown that one a while ago at work. We were told he was racking in an earth truck - the bus was supposed to be isolated, but they isolated the wrong one...

The permit system, or their lack of adherence to it has failed them on this occasion. Testing should have been conducted to prove the bus was dead before they installed the earth....

Haha, funny that when Holcim were bringing in their new arc flash procedures they showed the same one to us and said he energized a breaker. It is on a whole different section to all the other LV gear so might be HV. All our setups have overheads connecting to a tranny outside the MCC and only the LV mains running in.

Yep, wouldn't have taken much to prove it was dead - or not in this case...

As said racking in a HV circuit breaker. Now usually done by sparkies. We used to do the racking in and out as part of the isolation of mills years ago. The crack they make normally scare the bejesus out of you let alaone one going wrong like that. Hence all the PPE you must wear.
