    my left shoulders like that...

since I broke my clavical the muscles dont sit right when I work the shoulder hard and the muscles swell up they pinch the nerves so other muscles take up to relieve the pressure on the nerve which in turn pulls my spine an neck out of alignment and I wind up with huge pain in my upper left back and spinal area.

then its off for 2 weeks of low strength activity restrictions and daily massage sessions. (which usually hurt more than the problem itself)

Fortunately I can palm steer all the rovers and usually only change gears with the left hand.
