The symptoms may well be a combination of various problems, but the main one is probably the swivels or possibly the wheelbearings.
Jack up a front wheel - try to rock it side to side and up and down. Ideally there will be no movement. If there is movement, repeat with someone standing hard on the brakes (this eliminates movement on wheel bearings). If there is any remaining up/down movement, it is swivel bearings, and with that amount of movement there is an urgent need for repair or at least adjusting preload. Sideways movement will be steering linkage follow it up to find where.
If there is significantly more movement when the brakes are released, the wheel bearings need adjusting.
