Fuel filter ?
As it is in the middle of winter and cold in Canberra (-5 last night) I normally give the glow plugs two cycles before start but if I forget it will still start but run rough for a few seconds until the cylinders get a bit of heat into them.
However over the past few days the engine has been running rough after start for a little while even with the correct glow plug starting cycles.
Last night the car was left outside but I did not try to start until mid after noon with outside temp was about 10. After the glow plug light went out I started and the engine fired but immediately stopped - I went through 5 start procedures befire the engine continued to run with heaps of smoke coming out the exhaust - though not to be expected due to the number of failed starts. When it finally started it ran fine and when warm started first go. At no stage did I get any fault messages or lights.
I am open to suggestions of what the issue might/may have been.

Are you sure the glow plugs are still all functional?
In what way - as in blocked - not sure but it has only been in for 20,000km (half the recommended 48,00km service interval) so should be OK.
Hmmm - no but I am assuming they are still functional but I do not know. I see from the workshop manual that if the relays play up there should be faults logged so I will check that tomorrow - I guess I need to go through in detail and get the system tested.

It does sound like glowplugs, either they have failed or a problem with the glowplug circuit.
Sounds like they can be a pain in the TDV6 to get out without breaking.
Had a similar problem with one of our diesel Territories, turned over OK after 'glowplugging' started but smoked & ran rough for a few seconds. Turned out that a new battery fixed it after posters on the Ford Forum told me I'd have to have heads off to replace glowplugs etc. etc.........

I haven't checked anything yet but just went out and started the car at 2 in the afternoon - temp by the gauge on the dash was 4 degrees. One cycle of the glow plugs and the car started fine so yesterday's issue must have been something transient.

As a handy hint for when you do need to remove them, start the car and warm it up for a few minutes to warm up the block. The glow plugs come out easier then. I snapped a glowplug on a 300 TDi on the weekend, luckily from a stuffed engine I was stripping for parts. Bit hard to start it when it was already out of the car and cactus!

It may be this - it started fine today - however about 4 weeks ago I was replacing my front brakes and I noticed the fuel filter had a small leak - not enough to drop onto the ground but enough to smear the underside of the car with fuel. Noted that and as a service was due bought a new fuel filter ready to go.
The following week I replaced the gearbox sump and fluid but noticed the fuel filter leak was gone so for better or worse decided not to replace it. All good until the incident of false starts this week - but looking under the car the fuel leak is back so when the weather is good I will need to change the filter.
Got me thinking - when the car is parked flat in the garage the car starts but when I had the problem it was parked overnight on a slope on the grass. I am wondering whether the small leak allowed enough fuel to flow back out of the system to prevent starting until the lift pump primed the system.
Any way - we will see - I will replace the filter and if the issue continues then onto the glow plug system.
