At the risk of asking a stupid question - are you sure that your LT95 box has the Isuzu High range gears? It sounds like you may have the lower transfer case ratio from a V8? Is it possible that the box has been swapped in from a V8 at some stage?

Apologies if you've checked this possibility, but the description of the engine 'screaming' sounds strange.

Good point - the Isuzu may be an aftermarket conversion that did not include changing high ratio. I have no problems wanting a higher ratio in my 110 Isuzu with LT95 - it is quite pleasant at 110kph, and "screaming" is not a description I would have thought of. However, I would comment that sound proofing is a lot better on the 110 than Stage 1, including a noise deadening rocker cover.

The 4BD1 and LT95 from an '81 Stage1 that I put in my county had the 1.3:1 high range ratio in it from memory.
Pretty sure it was a genuine.

If you're just after lowering the revs at 100kmh and the LT95 is in otherwise good condition just get hold of some .996 high range gears and fit those. Much easier than a 5 speed conversion, and you don't even have to pull the box out to change them.
Add a turbo and some 255/85's and you'll be pleasantly cruising at about 2000rpm (quite a big drop from the 2600ish that you'll be doing now if you've got the 1.3 gears).
