I haven't got time for a full explanation (sorry) but my money would be on the little mechanism, inside the top of the injector pump, sticking or getting worn.

A drop of oil/grease might help.

It also might be worth checking your timing, condition of timing belt and valve clearances.

Make sure when draining the sediment bowl (located at rear of lhr wheel arch, next to edge of fuel tank) that you actually remove the bowl, not just drain it by undoing the bottom plastic draincock. I have seen many of them brimming with algae/ bacteria and sludge like jelly and water. This will be even more necessary after running a cleaner throgh the system as any muck in the tank mobilised by it will settle in the sedimenter and filter. Also check that the electrical connection to the injection pump isn't loose. Wiggle it while the engine is running to test that theory :-)

Make sure you don't have an air leak in the fuel line somewhere!

Could it be your lift pump ? Mine had failed leading the engine to run rough at times

A not-so-obvious one Justin, it has happened here.

Your suggestion could be extended to check all crimped connections in that circuit.

Looking from a different angle, perhaps a loose or delaminated intercooler hose ? Worth a look anyway.
