My 85 110 had it on the drivers side from factory as there were no holes for it on the passenger side.
The spare tyre mounted on the door did cover it a bit, if you had a carrier it would probably make visibility worse.

Mine was bolt to the roof rack and rear cross member only, no holes body panels.
Yes I use it with the folding step, and yes you could strengthen it, that said I am 100kgs and it has to date had no issues with that weight, very strong mounting top and bottom. Though I climb up and the stand on the spare tyre.
Sorry no idea where in Oz you could buy.
I just use an aluminium telescopic 2m portable ladder. I can access all sides - no getting up on the rack. Bonus is I can use it for other jobs around the campsite or at home. I've made a lockable carrier for it for between the spare wheel & wheel bag, so its easily accessed, or not even taken away if the roof rack is off for short trips.
I have to remove the roof rack to get the Deefer in the garage anyway...

Granted its not a ladder (and one is on my wish list) but its good for loading up the roof (above the wheels - otherwise you need to open the rear doors and step on the floor).
I have a full length Hannibal roof rack and Hannibal rear ladder. Ladder looked a bit light weight at first but it’s not moved since installed. Works a treat. Only consideration is learning to not have your fingers on the edge of the rear door when you close it, you jamb your fingers against the side of the ladder.
