Usually the problem is solved by removing the flange nut and filling the area with some good quality sealant. If the problem recurs, more investigation will be required, but it is unlikely.

Only if the threads are stuffed. Are they tacked on? It's been a while but I thought they were just pressed in, or held by a large clip.

To felt washer + goo or not to felt washer, and just goo - I have read both opinions...
I just use goo.
If the output flange body (576627?) is intact, best just to replace the shaft bolts/nuts?
If there's no reason to replace it - seal surface worn or chipped, or splines worn - don't worry about it.
