About every 6 months for me, only if the comp is starting to hang up and drive me nuts. If it's going fine I don't usually worry about it. I only use the thing for web browsing, which doesn't tax it much.

Usually every 3 months or so after a clean out and registry clean up.

We actually did not own a computer in the 1990s.

The firmware, cache, capacity and seek, read, write speeds of modern drives makes this less of an issue than before.

File fragmentation occurs no matter what the operating system. There are Mac and Linux defrag utilities available. Fragmented files will always be read slower than unfragmented ones. As stated before, the speed of newer hard drives makes it less noticeable.

Pretty sure Win7 defrags automatically (sets up a schedule on install) at least mine does and I don't remember stetting it up
