Highly likely that one of the pipes on top of the tank has worn through - it's a relatively common problem and there was a service bulletin or recall for it some time ago. I had it happen to me on a trip at the base of Mount Hotham - managed to drive out to Hotham and had a tow truck take the vehicle to Wodonga for remedial work.

You need to lift the carpet in the rear and remove the tank inspection panel to assess what needs replacing - not a hard job I believe.

    Cheers,

    Franz

You may also find the pump locking ring has come undone or the hose clamp and joiner has come apart between the tank and the filler pipe.
    Cheers
