I had very similar issues with my Defender TD5 and the culprit turned out to be the little one way valve in the head of the fuel filter unit. It is a fairly cheap part and takes little time to change.

Regret to report that my mighty D2a (03, white, Td5) is acting up.
Once started, the car runs perfectly with no issues.
However, it will intermittently reallllly struggle to get going. To get it started I hold my foot on the accelerator and just leave the key turning the engine over and over and over. Sometimes it will take 30 seconds to start actually firing. The problem seems to be worse when the fuel level is low. Does not seem to matter whether it has been sitting for a little while or not (although I may be wrong here, and when I say sitting I mean only for a day or two).
I believe it is a air in fuel/fuel delivery problem.
I have had this problem in the past (over one year ago), and in trying to diagnose/correct I have replaced the following:
1. Air bleed valve in fuel filter bracket.
2. Fuel pump.
3. Fuel pressure regulator.
4. Injector seals.
5. Injector wiring loom (although I have noticed oil creeping back into ecu).
6. Hose (with loop in it) coming from the fuel tank, as it had a kink in it.
7. I have put diesel algae killer in the tank previously.
The thing that seemed to work well was changing the kinked hose coming from the fuel tank, and I have not had issues in over a year.
The car always starts, eventually. Has never failed to start. However, I am thoroughly sick of this issue.
Any advice/insight would be UNBELIEVABLY appreciated.

On the fuel filter housing, the front pipe closest to chassis, remove the fitting from housing.
On the end of the fitting there is a rubber check valve. remove the check valve from fitting and then separate rubber tip from plastic end.
You will notice the hole through the plastic reduces drill this out to the same size. I think from memory it is a 1/8 but check.
Reassemble Job done
This is the return line from the engine and reduces the return line pressure. It slows the diesel ingress into engine.
I check oil level on TD5's before servicing and it is surprising how many fill the sump. Its more common on the later Td5's with green injectors.
Remember reading this sometime ago, might be of some help.

So I tried drilling out that plastic piece (thanks Woko)
Should the fix be instantaneous?
I have started it probably 15 times since I drilled it out and it has only struggled a few times. It also feels like it isn't struggling as much when it does struggle. This seems to be an improvement, but it may be confirmation bias.
Any other suggestions?

Very interesting. I have a similar fault condition when the engine almost starts but I let go of the key too early. I have to let it sit of a few minutes before it starts like nothing ever happened.
Don't do that as long as the engine didnt run out of fuel cos you might flood it, as long as there was no intervention on the fuel delivery system there's no need to touch the throttle whatsoever, also if it doesnt start right away without any throttle after a 5 pedal purge sequence better check the compression... I hold my foot on the accelerator and just leave the key turning the engine over and over ...

If the engine compression is bad then surely I would have other symptoms?
Or do you mean fuel system compression?
The fact that it is worse when low fuel would indicate it is a fuel system issue?
I have put a tank of diesel in it recently and have not had any starting issues (although fuel level still quite high).

I meant engine compression though if it's fuel level related remove the tank's cap next time when it makes tricks and if it starts easyer check that pipe from the attachment cos it used to choke at the pointed section and one of the symptoms is hard start at low fuel

I have actually replaced that piece of hose and it did correct my issue for about 12 months.
Now I am having the same issue; I suppose I ought check that hose again?
I ended up replacing the kinked piece of the hose with some braided hose and I am sure that the braided hose hasn't kinked...Maybe it has kinked elsewhere?
How would I go about replacing that hose anyway?
What is the cause of this kinking problem?

I dont know the cause maybe cos the vapours are quite hot nor about fitting but if the pipe is not choked there it might be some kind of valve in that vent side nr. 23 though i'm just presuming, what's certain is that if the breather system doesnt work well it can cause all sorts of issues hard starting at low level included, here's the description from RAVE maybe you figure out more:
