How the hell do you get tapped under the car changing a tyre ? Why would any part of your body be under the car?
Changing a tyre, thinking 'maybe I'll just check the tie rod ends' or something like that. A tragic outcome for maybe a couple of seconds of bad decision making. I won't stick my head under a car without a stand in position first if one of the shells has to come off. Luckily, I can do almost everything on the 101 without jacking the vehicle up, I can scoot straight in underneath it.

Just guessing, but maybe he had to jack the car a bit higher to get the fully inflated spare onto the hub.
If the jack was under the axle he may have had to climb under to fit the handle to wind/pump the jack, it may not have been accessible.
That's why I always put the spare or flat wheel under the lowest part of the chassis. If it then falls it will be unlikely to crush you as it would also be unlikely to carry a chassis/axle stand with you, sad to hear, Regards frank.
Just two weeks ago I had a mishap which required me removing two wheels.......one wheel nut rolled under the trailer, it stayed there until I positioned the wheel under the axle.......without thinking I suppose I could have crawled under.
But we are only guessing the actual event.......
