    Yep. Then, in 2018 a SouthWest Airlines jet had an engine rotor burst. Debris broke a window and Jennifer Riordan was sucked mostly out. She wasn't so lucky. Not to mention Aloha Airlines Flight 243 in 1988, although how anyone survived that one is beyond me.

    EDIT: Probably neither of those were due to wrong screws though. Missed the context at first.

    I flew that route with Aloha not long before that happened, I have never experienced any piece of machinery that felt like it was going to shake itself to bits as that aircraft ☹️

    I reckon Rex bought that plane and I flew out of Mascot on it, last year.

    I too flew that route with Aloha a couple of weeks before the event. Thought it was strange that the plane never leveled out during the flight. The plane flew all the way with the front higher than the back. Was a down hill walk to the toilet at the back and up hill on the return journey

    Probably trying to keep the fuel levels covering the pickups..

    That Aloha plane (and presumably others in the fleet) had 90,000 flight cycles for only 35,000 flight hours. The conclusion was poor maintenance by Aloha, on top of manufacturing defects with early Boeing 737s, and failure of FAA to mandate specific inspection for these known defects (metal to metal bonding).
