Lots of deadsticks - usually OK except in my helicopters.
I think only one (aircraft) radio failure and flyaway of a 10 ft glider. Lots of boat radio failures. Ever heard a .40 racing engine revving its guts out at over 25,000 rpm when the boat has run up the bank and the prop is unloaded - then the silence when it breaks a rod and suddenly stops.
Many, many pilot errors.
But was flying from 1960 through until 1989. Gawd, I haven't flown for 20 years.

I was cleaning the house today (wife home from holidays tomorrow). I went to put on a CD and remembered, when the CD door stuck, that this CD player has been faulty since new. I'd forgotten all about it. I bought it on Norfolk Island so it's a bit hard to take it back and the warranty doesn't apply in Australia.
And the new Pentax SF7 SLR camera that went up in smoke (literally) the third time I used it back in the early 90s.
Or the new Pentax Spotmatic I bought in 1968 that went back to CR Kennedys many times for a jammed shutter never to be fixed. Although the post office did drop the camera and bend the case around the pentaprism on one of its return journeys from CR Kennedy. The camera still jams the shutter to this day.
Or my Yamaha motor bike that broke a crankshaft shortly after I bought the bike.
How come my marriage has been OK - nearly 40 years?

Another good day.
I went for a Two Wheels Forum ride today - about 600+ km. We stopped for lunch at Sandy Hollow (near Denman).
I ordered a caramel malted milkshake and a hamburger.
The malted arrived with the comment "It doesn't have much caramel because the bottle was nearly empty."
Umm, maybe you could have told me so I could change flavours.
Then the burger was delivered - without a meat pattie! Gahhhh!
When I got nearly home I refuelled the bike. It started then stopped after fuelling. Now it won't start. It could have been worse, I could have been 200km from home rather than less than 1 km. The bike is fuel injected so I hope it's not dirty fuel.
