    Ive seen something like that in an f88 mag that hadnt been assembled properly after cleaning, the lift spring jammed after about 5 or 6 rounds and lift platform stayed at the bottom, carry out the IA, faulty mag, change mags carry on the contact.

    end of the day and when the mag was pulled out of the bum pack for final clearance half a dozen rounds had stayed in and laid over so they were up and down the mag instead of in the normal fashion.
