Can be a symptom of any condition that causes persistent low levels of blood oxygen, usually lung or heart disease.

Can be caused by deficiencies. Without seeing what you are talking about it is hard to tell. Could be SPO2 (oxygen saturation in blood, easy test with an SPO2 meter, should be above 95%, anything below and you may have issues, normal is 95-99%, rare to see some one unless on O2 at 100%). It can also be caused by nutritional deficiency. There is more, such as some obscure diseases, but that would be speculation. Next time you see your doctor ask them to run a few tests. Could also be done from previous nail damage.
If you know any paramedics they usually have an SPO2 monitor in their ambulance.

its caused from working on landrovers, the gunk gets under your nails, its just one of the landrover syndromes
Usually found just under the head. I've always assumed it was from some sort of clamp that held the nail as the head was forged.
