Thanks DiscoDB, I had read that thread but went through it again in case I missed something. One failure in the many, but do not know the details. Still unsure but I guess time will tell.

Most likely not maintained properly and dirt in the latching mechanism - or rust.

Sounds like more cases, but related to leaving fitted when not using which would be a problem in the UK with the level of corrosion issues they have. Different style but the way it locks into place is the same.
Don't like the thought of my standard hitch falling out so always do a finger check to make sure the locking pin is fully out or in as the case may be. Made easier to do as I have the spare on the back instead of underneath. Plus when I was first aware that falling out had happened I checked the thing and found the top most part which goes up inside the receiver wasn't flat across the top. I ground it down and it may be my wild imagination (usually used for other things) but I believe the locking pin came through the locating hole a bit further.
Other wise I'm quite happy with the set up and always remove it and wash out all around it after a trip. Although I still can't understand why LR couldn't have done what everyone else does and have a straight square receiver but it's all part of LR ownership I suppose. :)

Would have been good if LR originally had a receiver that could have been bolted on. Same mechanism but 2 lugs and bolt holes so you can secure permanently but could easily remove if you needed to.
I guess the UK version had to be removable because it looked so embarrassing - heh heh.
The aftermarket design like the Mitch Hitch is brilliant - I am just put off having a lump of steel projecting out the back.
