Possible problems which could cause this:-
1. Incorrectly assembled synchromesh unit
2. Bent selector fork or worn pads on the ends, or loose or incorrectly adjusted on the selector shaft.
3. Selector detent spring weak, broken or missing.
with the top off the overdrive select overdrive and ensure that the selector fork is positively located by the detent ball and that in this position the dog is fully engaged. If it is not, it may be due to maladjustment, or excessive wear on the pads on the fork, or something stopping it engaging, usually a stray bit of synchromesh spring. Also note that any end play on the mainshaft will lead to the dog not fully engaging, and you should ensure there is no endplay. Sloppy bearings may also lead to disengagement.
