There's not too many things can go wrong with a clutch , If your satisfied the hydraulics are working OK . If you're pulling the box back again , Check the pressure plate for damage , check the clutch plate , shake It to see if the springs are loose . While your at It check the spigot bearing in the fly wheel , have a look at the fly wheel as well ? Next thrust bearing condition , and is It sitting right , after that clutch fork . If your happy that all is well there then , It a problem in the box , input bearing ? At this point if It was me I would be like you , asking for help . There's nothing worse than bad advice , I do hope you get to the bottom of your problem , It sounds as though you deserve a break , Good luck , Jim ...

Grab hold of the throwout bearing and slide it back and forwards on the guide.
If it has a tendency to grab or stick then the alloy guide needs some work to remove the wear step or replace with a new cast iron one.

What stage were you at when the jack slipped? Did the gearbox move much? hang up on something? fall far?

Probably just a dry spigot bushing - in the end of the crank, they will make one hell of a noise after awhile when letting out the clutch but it's not hurting anything. Other than that, could be the dual mass flywheel on the way out, these are generally replaced whenever the clutch is replaced.
