You will probably find the actual switch at the back of the barrel is loose,possibly one of the tags that holds it in place has broken or lost tension.

Well, I can finally say problem solved.
Since replacing the ignition switch, I have had no repeat of the issue.
Having to use a different key for the ignition switch is not my most preferred outcome, but I really do not think it is worth having the lock re-keyed at this point.
The ignition switch itself was indeed the culprit. It literally fell apart when I tried to remove it. Genuine Prince of Darkness part. There is evidence it has been repaired before too- presumably because the P.O. did not want to do what I did to resolve the issue.
New part is Valeo. Hopefully it is a better quality. Only time will tell.
