I once had 4 tyres fitted by a so called tyre expert. After running over an Echidna, one of the tyres went flat. It took a 1 m length of pipe, loaned by the local farmer, to undo the wheel nuts. The next day I went back to so called expert and asked them to remove the other tyre which still had Echidna spikes in it. Their own air operated wheel tightener, which they used to put the wheels back on, could not undo the nuts. The manager said to me "who put these on?", I said, " you lot did" . They had to use a length of pipe too.

Tank is correct as to causes - fairly simple to replace

I've seen it a lot in the third world (PNG) where a combination of lack of knowledge about wheel nut tightening (either to loose or too tight) and severe potholes seems to cause it to occur regularly - many police LC traybacks are down to two functioning studs on the rear before they are repaired.
