Any chance of some more information on what the oil cooler/adapter plate is please?
Coincidence? And this is NOT a dig at our LR service provider.
In my street there are 2 x MY04 D2a TD5 autos, both with approx 150K. In the last few weeks both have had to have their engine oil coolers/adapter plates replaced due to breakages during servicing (apparently failing is not all that uncommon), and both have subsequently blown their turbos (1 near Docker River and mine south of Perth).
I did buy some extra Lotto tickets last weekend for my 65th birthday, but not yet a millionaire and still hopefully waiting for my luck to change.

There is a small engine oil cooler (approx under/alongside the exhaust and turbo?) which is connected to the block by an adapter plate. Apparently the plate can relatively easily be damaged (overtightening?). Sorry, can't do pics - engine is 20 km away and I've lost my digital camera.

Sorry guys, can see how it could read that way, but my intent was misunderstood/not explained - the coincidence was the commonality of the two sister cars (including model, location, age, mileage, the oil cooler work), but not that the oil cooler work led directly to the turbo failure.

I reckon there is a very good chance the oil cooler work has lead to the turbo failure.
Repairing the oil cooler necessitates removal of the turbo for access. It seems odd that your mechanic as had to fix two broken oil coolers on two D2's which have both subsequently blown turbo's. That would make me very suspicious of the way they have reassembled the turbo oil feed. If they have taken a short cut and used gasket in a tube rather than replacing the gasket it is possible that the oil feed to the turbo bearings have been blocked by excess "goo" resulting in bearing failure.
I'm assuming you have taken the vehicle back to the same mechanic, who is unlikely to tell you if the failure is the direct result of their workmanship, so it will be put down to bad luck/coincidence.
All seems a bit strange to me.
I am only aware of Oil coolers suffering when the Cooling System has been neglected(multiple issues). So possible servicing issue to begin with. Why was your cooler replaced and the other vehicle?
As Tombie said no reason to touch during servicing.
2 vehicles , 2 coolers, 2 turbos, same service centre ....bit more than curious.
As Offtrack says also crappy silicone gasket, or perhaps the methods used to ensure cleanliness during disassembly, repair and replacement are suspect.
What oils/ grade are being used during service?
What do you mean by a "blown" turbo?
Are the turbines rubbing, seized or is it leaking oil etc?
To be fair to the service mob, some more specific accurate info could be beneficial to all.

Do you have a RAVE DVD ?. Just looked up the oil cooler assy. on my RAVE for my D2, there is no adapter for my earlier model, the oil cooler bolts straight onto the block, with 7 bolts.The oil cooler is not part of a service routine, the usual suspect for maintainence is the stepped hose , out of sight , behind the oil centifruge & oil filter, under the exhaust manifold. Did the dealer take the turbo off? If so, for what reason? Bob

"Interesting" comments (and conclusions) from all. I don't doubt the integrity of my service provider, and they are a well regarded, small LR specialist operation, but yes accidents/incidents can happen.
Yep bob10, I have the Rave CD bought ex Dave some months ago (but not having read many parts in detail, sometimes find it still talking about earlier D2 stuff, whereas ours are upgrade D2a MY04 versions).
Following bob10's comment, I've just re-read my invoice and may have misunderstood what was done. The list says in part I had the following replaced - oil cooler assembly TD5 - PBC500230-A ($750 + GST) and oil filter adaptor -LPY100160? ($300 + GST), which of course are 2 separate and not connected items, so looks like you have hit it on the head bob10.
Can't speak for my neighbour, but I had my exhaust manifold taken off for me to fit my OZ Thermoguard EGT probe and to deweb and restud, so guess that is when the broken adaptor/assembly issue arose.
