I've worked out my thremo fan does not engage at 110 degrees (with a little help from sierrafery) but does come on with the A/C. This maybe part of my problem, but don't think it is entirely to blame.

If your not hearing any roar then it would indicate a buggered visco. I had the same thing on my old D1, when under heavy load up hill low range it did not roar, after a new visco was fitted same situation and it roared loudly.
Lots of people driving around with buggered viscos that don't realise it. The paper roll trick doesn't duplicate actual under heavy load situations
