From what I saw.
The chain was obviously not stacked too well below decks.The man operating the clutch kept backing it off to get the chain to flow.Once it was flowing it was gone and he couldnt brake it.Didnt need to be deep,it would have fed out just as fast in 20m as 2000m deep,the chain is heavy,the anchor or chain length has nothing to do with it

Getting the bloody things back up with out a capstan- Wish I had video of that effort. Triple blocks set pulled by second set of triple blocks. 80 guys on the line of the second set port side and the same set up with another 80 of us mugs starboard side. Took just over two hours in Jarvis Bay 1986. Back in Sydney for a beer by 1am
re the navy's OH&S. Yanks might get away with that but my balls would have been chewed off if I stood near a line or god forbid straddled a line or cable

- What an anchor & chain like that would cost? Cost- no idea LOTS
- What an anchor & chain like that would weigh? HMAS Sydney was 2.7 t. One on one of my ships was bigger at well over 3t The anchor in the video looks bigger to me so I guess about 5t. The cable looks the same size so much more than the anchor for every 2 chains worth

- How the braking mechanism works? Poorly - sorry could not help myself
- How the braking mechanism fails? (Was it user error or mechanical failure?) I have seen three fail, Sadly it was always on the way up!! Old- wear. heat stress, poorly maintained and once poor engineering
