I suspect the roofrack may have had more than its rated load?
Also reminds me of a friend who had a kayak arrive in his car via the rear window after he stopped at an intersection and the car behind made a sudden stop.

Obviously needs to keep his roof rack fittings a lot tighter. Maybe they vibrated loose on the corrugations. I've had that happen, but I didn't loose the rack.
I remember once seeing an accident in which a small boat on a trailer had come loose during a sudden stop and speared through the windscreen of an oncoming vehicle, killing the driver.![]()
