Northern Lander summed it up (I think). The shaft has a radius on it so a collar is needed with a chamfer inside to fit over the radius giving a flat surface for the bearing to press against.
A radius is used on the shaft rather than a sharp edge which would be a stress raiser.
Your axles should already have the collars pressed in place. Hang on, in Northern Landers second picture the 'washer' on the RHS is the offending item.
A picture of your axle would help.
