My Isuzu C240 Series III came with a high speed transfer box.
May have been an Australian version because one of the gears was welded up (centre from one gear, outer from another) picture in post #59 of this thread Isuzu C240 powered Series 3
In post #45 you can see the modification to the transfer box idler shaft position, I think it appears in a few other posts.
4 Wheel Drives in Melbourne list them but I think they are from Ashcroft. In their old catalogue it's $935 but that included the modified case.
The previous owner of mine claimed it was a bit slow pulling away (maybe because it's a small Isuzu NA diesel !) but it had 'long legs'.
Mine isn't on the road and since getting a replacement flywheel made (earlier in the above thread) I've only driven it down our driveway so I can't give any first hand feedback yet.
