    So in theory you should be able to rule out the EGR and the head.
    Yet, IMO for that much coolant to be entering the cylinders it must be coming directly through the inlet ports or fuel galleries, it should have hydraulic locked the motor. Despite the head replacement, it still seems the most likely.
    Perhaps some mods have been done to the head like studs replaced which have been drilled too far.

    Have you pressurised the cooling system in situ? This much coolant loss under pressure with the injectors out should be quite quick to show itself under pressure. From your descriptiion the failure seems to occur around the time the thermostat should be opening.
    Just curious how is the fuel cooler? Is there water in the fuel system as well?
