What would blow off a TDi300 rocker cover??

I was asked this, a guy had his rocker cover blow off his 300TDi.
What would do this Excessive crank pressure? Leaking rings?
Diesel in sump oil causing high level and crank case pressure?
    opinions welcome

How about defining this rather emotive description of "Blown Off"? Blown off should indicate that is no longer in its original place ,like all 4 bolts have snapped and the cover and the sound cover , breather assembly are no longer there. or is it that the sealing arrangement has simply been forced out ot position ? for whatever reason ?Sounds like the standard blown head gasket to thr push rod tubes/holes

The email i received was blown off and snapping a rocker cover bolt in the process. I have seen the vehicle myself

RDX, C4, ANFO, Cemtex, Detcord, black powder, smokless powder, cordite. Just some of the things that Could blow the rocker cover off of an engine.

Had this happen to a boat in the marina I work at and the engine was essentially on fire internally and kept on running . Couldnt shut it off . A mechanic was there at the time and managed to shut it down somehow but I dont know how .
It was running seemed like flat out and smoke everywhere and I mean everywhere . Was a perkins diesel !
