Had the same on a Territory diesel but it got caught behind the camshaft pulley.

TDV6 HP fuel pump pulley problem (not Land Rover)

Dealer had stuffed the belt replacement some time before so I managed to get them to pay for the repairs. Next the tensioner 'fell off' (some years later). Managed to get them to pay for this as well.

Maybe it's worth checking if the guide on the pulley is loose when servicing......?

I definitely will be checking from now on when I change them. It had done 80000km since the belt was replaced. Don't know if it would of been found loose though. All the spot welds are still intact on the pulley. The guide had cracked around the spot welds

How did you get the pulley off ? , i had to replace my LH exhaust camshaft and that pulley was impossible to get to.
Ended up removing cam complete after breaking the plastic cover.

Getting it off wasn't the problem getting it torqued was the hard bit. I made a tool to hold the pulley and another to do the bolt up. I could get a 3/8 torque wrench in there to do the torque, but needed 2 foot bar to do the angle. Took me all day to do it but that was also making the tools. Not a fun job but I would not take the body off to do it again if needed to. Here's a photo of the tools. 20210118_184247.jpg

I made similar tools so i could torque it on the work bench, fitted the cam with pulley mounted and had to drill a couple of holes in the plastic to do up the cam cap bolts and make sure the new seal seated properly. Real bitch of a job especially refitting the lower most bolt that holds the belt cover to the head, barely enough room to get fingers in there let alone a ratchet or spanner.
Belt fitting was the easy part.

They did the cambelt & idlers at the same time as the HP pump belt. Don't know what they did wrong when installing the pump belt but thankfully no cambelt issues. Car has done over 50,000 since the cambelt was changed.
I did ask the service manager what would have happened if the cambelt had been the one to fail. He told me that they would have fitted a new motor !
Then I asked if this was something they'd had to do in the past.....He said he'd been in the industry for some years and he knew of at least 7 cases where a new engine had to be fitted.
