Tusker seemed to think his one failed as the maxi bronze bush wore through which allowed it to move about, then the stub rubbed on the tube and it let go,

I have seen a few failures like the Italian one on the earlier inners, what happens is the root of the spline runs into one or more of the ball tracks and this acts as a stress concentration, then a crack starts and runs across the ball valley, the later inners had a 3mm undercut to reduce the stress concentration and we have not seen any of these fail, the current design uses a larger ball and ball PCD which reduces stresses on the inner and gives a bigger wall thickness on the inner, not seen any of these fail yet either,
