300Tdi heads & injections

I'd like to get a few opinions and tips on 300Tdi maintenance issues.
Our '97 Dscovery has about 255,000 km on it now and I'm thinking of shouting it a set of new injectors (overall economy has been trending very slowly downwards over the last couple of years). A few years ago I had one injector out (long story) and it was very tight in it's hollow in the head and there was quite a lot of dirt, rust and gunge around the barrel of the injector.
So, what's the recommended method of removing the injectors without damage (in case they are still quite OK). I've made-up a slide hammer to attach to the top thread so I can knock them out without twisting but is this the best method? And after they're out, what's a good way to clean out the hollows in the head without all the gunge falling into the cylinders?
Next question is the head gasket. A lot has been said and written about head gasket failures on 300Tdis but do they all fail eventually? - or do we only hear about the minority that do? Is a "pre-emptive strike" called for? That is, at 255K km would it be a good idea to replace the head gasket because it's probably likely to fail sooner or later anyway? Or do many (most?) of them hang in there for much longer? Your thoghts (especially from those 'in the trade' who see plenty of examples) would be much appreciated.
