As Dave said, TC lockup clutch clearances could be way too high, but the later 4HP22's had a 'Hysteresis' valve ( A fancy kind of thermostat) in the valve body. This has the primary function of only allowing lockup once the trans has reached a certain temp. This valve is quite prone to sticking, especially with dirty/ infrequently changed fluids.
