If you have tried all the earthing ideas and alternator and battery it could very well be the starter motor itself. I repaired my starter motor after chasing a cold starting problem. But first I chased my tail for ages doing the injectors and all sorts of things before I found it to be as simple as the starter motor needed some more brushes.

Correct, but the brushes may have lasted longer if the earthing etc was up to scratch. Poor earthing (From factory) and low voltages can cause horrendous arcing at the point where the brushes run on the comm, this is one of the main causes of excessive brush wear. As far as I know My starter has been untouched for 550K, still in good shape. I run huge cabling and direct earth connection from batt neg to engine block and then body/ chassis.
