Some recent experience in the USA has found that a heavier flywheel improves the smoothness of the 4BD1 - some have used 2 flywheels as a way around gearbox conversions to the Isuzu flywheel housing.
I have mine out at the moment and intend to look at the practicality of increasing the flywheel weight.
The very similar Cummins 4BT has a much heavier flywheel (and pulley on the front).

Use an R380/1.2:1 LT230 and you will be very happy. Mine has now done over 65000ks since the conversion - including 4 major trips to central Australia - just got in from the Eyre Peninsula/eastern Flinders/bottom end of the Strezlecki/camerons Corner/bullo Downs etc. Hundreds of kays of soft going nil problems.

I have had an LT85 and currently drive with a LT95. I would go for a 5 speed.
I know where you can get a LT95 bell housing and possibly a 4BD1T that had a Garrett manifold similar profile to the one fitted to the army 6x6 (except they have IHI turbo bolt pattern).

Having rebuilt LT85's and LT95's, I am sticking with my LT95 . The gearing with 33's and 1:1 high range and 3.54 diffs is perfect, how fast do you want to go??? I just towed 2 tons back from the east coast and had to keep backing off on the flet because I get a nudge in the ribs when I get over 110km/hr
Around town it is fine, I get 11 to 12 l/100 city and 15/100 worst case towing 'aggressively'.
With a taper roller bearing T/C conversion the LT95 is the best prospect for longevity and strength IMHO.
